If i am going to buy a chain online and it says 23 inches does that mean 23 while hanging from the neck?

or 23 inches while it is unconnected and put straight??

Its how long the entire thing is, unconnected.
So if you want to know how long it’ll be, just measure around your neck and subtract that from 23 inches =)
